The first month is when clear aligner treatment goes from new and slightly strange to a routine you barely think about. Knowing what's normal makes it much easier.
Fitting day
Your provider may bond attachments to some teeth and create small spaces between teeth if your plan calls for it. You'll practice putting trays in and taking them out, and you'll go over your wear schedule and cleaning routine.
Week 1: getting used to them
- Expect pressure for the first day or two — a sign the trays are working.
- A slight lisp is common; reading aloud helps your tongue adapt faster.
- Tray edges or attachments may rub your cheeks; ask about smoothing a rough edge.
- You may notice more saliva at first. It settles within days.
Week 2: building habits
Removing trays for meals becomes automatic, and the lisp usually fades. This is often when patients switch to their second set, depending on the schedule. The first days on a new set may feel tight again — that's expected.
Week 3: the routine clicks
Most patients now wear trays 20 to 22 hours without much effort. You'll know your go-to meals, where your case lives in your bag, and how to fit brushing in after lunch.
Week 4: first changes
Depending on your plan, you may start to notice small changes, especially if you had gaps or mild crowding. Many patients take a photo each week to track progress.
Common first-month mistakes
- Leaving trays out too long after meals
- Sipping coffee or soda with trays in
- Wrapping trays in a napkin at restaurants
- Cleaning trays with hot water or toothpaste
- Switching to the next set early to 'speed things up'
Your first check-in
At your first follow-up, your provider checks how the trays fit, whether attachments are intact, and whether teeth are moving as planned. Bring your aligners and any questions.
